A simple blood test is usually all that is needed for your doctor to determine if you have low testosterone.

A low testosterone blood test is necessary in order to properly diagnose you with low testosterone, also known as “Low-T.”

Anyone over the age of 35, man or woman, can be suffering from the many debilitating symptoms of low testosterone. Signs you may have Low-T include:

  • Increased fatigue, lack of energy
  • Loss of lean muscle
  • Increased fat, particularly adipose fat (belly fat)
  • Sleep apnea, or other conditions of disturbed sleep

Laboratory Tests for Low Testosterone

The basic laboratory test for diagnosing low testosterone is a blood test. A simple blood test can reveal your testosterone level. The normal range is 300 ng/dL to 1,000 ng/dL. Your testosterone levels fluctuate throughout the day.  Because they are normally highest in the morning, your doctor will probably want to do the blood test in the morning. The morning is the best time to receive an accurate testosterone blood test.

Other (Non-Laboratory) Tests for Low Testosterone

In addition to the laboratory blood test for your testosterone level, the doctor will want to give you a complete physical exam before providing a diagnosis of low testosterone.  He or she will look at:

  • Your amount of body hair
  • The size of your breasts, testes, scrotum, and penis
  • The consistency of your testes and scrotum (are there any lumps?)
  • How well you can see to the side (loss of peripheral vision may indicate that you have a pituitary tumor, which can cause low testosterone levels)

There are home test kits for low testosterone. However, these use a saliva swab to test your testosterone level. A testosterone blood test is far more accurate to diagnose you with low testosterone. Blood testing for low testosterone is used not only to diagnose age-related testosterone loss but any abnormality of your testosterone level. It is highly recommended that you avoid any non-laboratory or "at home" tests for low testosterone.

Any doctor can perform a testosterone blood test. However, if you suspect that you have low testosterone, it is best to be tested by a doctor or clinic that specializes in treating age-related hormone deficiencies for men.

What Does the Testosterone Test Measure?

Why Would I Get a Low Testosterone Blood Test?

Your doctor will want you to have the testosterone in your blood tested if he suspects that you have low testosterone. There can be several causes of low testosterone, but the most common is the drop in testosterone that occurs as men age. If you are a man over 45 and exhibiting any of the following symptoms, your doctor will likely order a testosterone blood test.

  • Erectile dysfunction (ED) or loss of sexual desire
  • Sleep disturbances
  • Decline in memory/cognitive function
  • Decrease in body hair
  • Fatigue or lack of energy
  • Increase in body fat, loss of muscle
  • Depression, anxiety, and mood swings

The most common cause of low testosterone in men over 30 is age-related testosterone loss.

How Is the Low Testosterone Blood Test Performed?

There is no special preparation needed for the blood test that doctors use to see if you have low testosterone. Over 95% of the testosterone that circulates in your blood is what we call "bound" testosterone. This is because it is bound or tied to other things in your blood, like certain protein molecules.

A much smaller amount exists as free testosterone. However, both are what we call "bioavailable" or your "total testosterone."  The typical testosterone blood test will look for your total testosterone. In some cases, to provide a more precise diagnosis of your body’s ability to produce testosterone, your doctor will also order a "free" testosterone blood test.

What Will Your Results Tell Your Doctor?

The results of your testosterone blood test will tell your doctor if you have a normal or a low level of testosterone in your blood.

The so-called normal blood level for testosterone changes with age, so your age will be one of several determining factors of what is considered normal "for you." Also, two men of the exact same age can have the exact same level of testosterone in their blood, and one of them can feel just fine, and the other may have symptoms of low testosterone.

This is why the normal testosterone level in the blood is expressed in a range. This is also why doctors take other factors, such as your medical history and your symptoms, into consideration before making a diagnosis of low testosterone.

It is unusual for men under 30 to have a significantly below-normal testosterone blood test result. Such a result in men under 30 could indicate:

  • A disease or condition impacting the hypothalamus or pituitary gland
  • Certain genetic conditions that are known to cause decreased testosterone output, such as Klinefelter’s disease, or Prader-Willi syndrome
  • A disease, injury, or condition that impacts the testes
  • Chronic hormonal diseases such as diabetes

However, if you are over 30, and your testosterone blood test indicates that you have low testosterone, then it is most likely age-related testosterone deficiency syndrome.

The safest and most effective treatment for age-related testosterone deficiency is testosterone replacement therapy. If your symptoms are indicative of low testosterone and the results of your testosterone are below normal, your doctor will probably recommend a program of testosterone replacement therapy.
